CONSENT TO COSMETOLOGY SERVICES

Dear Beloved Customer,

 

Please sign and date the following agreement before receiving cosmetology services at Designer Nails and Spa.  If the services are being performed on an individual under the age of 16, that individual’s parent or guardian must sign and complete the information in the space provided, and the following statements will be read to refer to the individual under the age of 16.  Thank you!

I, the undersigned, hereby acknowledge and consent to the following terms and conditions for the cosmetology services to be performed at Designer Nails and Spa:

I agree I am 16 years old or older (or that I am a parent or guardian of an individual under the age of 16) and acknowledge that I choose to receive services (or that I chose for my child or ward to receive services) from Designer Nails and Spa and that I have voluntarily signed this consent form.

 

I acknowledge that I am receiving services from a licensed professional and will conduct myself appropriately while in the salon.

 

To the best of my knowledge, I have no health condition or allergies that would prevent me from receiving nail services.

I grant permission for Designer Nails and Spa to use and/or apply any necessary products as part of the service I am receiving.  I understand that nail products contain reactive chemicals which may result in allergic reactions in some individuals and hereby assume all risk of personal injury which I may sustain as a result of the application of any of the products used, or the manner of service rendered, including the application of chemicals and use of appliances or implements including clippers, nippers, files, or other tools and materials.

 

I acknowledge that no guarantee has been made about the results of my service and that the service is satisfactory and final after payment is rendered and that no refund will be provided; however, if I notify Designer Nails and Spa within 3 days of service of any problem with the service I received, Designer Nails and Spa will do our best to fix or repair such defect without additional charge.  I agree that I may be charged for any changes, fixes, or repairs if I did not notify Designer Nails and Spa within 3 days of original service.

 

I understand that it is out of the scope of the nail technician to diagnose health conditions or to give any medical advice.

 

I authorize Designer Nails and Spa to take photos before, during, and after my nail service and authorize the use of these photos solely for the purpose of Designer Nails and Spa internet and portfolio publication, social media, or promotions and advertising.  I acknowledge that the Designer Nails and Spa is monitored by a 24-hour video security system and that I may be recorded at any time while on the premises.

 

By signing below, I am aware of and acknowledge any unexpected results or issues that may arise with the nail services I receive.  I am fully aware of any risks involved in the services I receive, including any adverse reactions or allergies.  I agree to voluntarily release, forever discharge, and agree to indemnify and hold harmless Designer Nails and Spa from any and all losses, claims, injuries, or damages, sustained or incurred by me arising out of or related to the services and/or treatments rendered by Designer Nails and Spa or results thereof, including but not limited to any claims arising from the treatment of my nails, the application of any nail products to my nails, including, but not limited to, claims based on any allergic reaction, damage or injury to my nails or skin, or any other claim and shall be responsible for all attorney's fees and costs incurred as a result of such claim.
